Fan not working

I see the following from journalctl:

I just added a fan and re-loaded with:

I’m using a “Raspberry Pi Active Cooler”

I’ve tried testing following these directions: Fan | RUBIK Pi Documentation

Any idea what might be wrong? One fan is mounted but I unplugged it and tried another fan also didn’t spin up.

FIXED:
Fan not working: I found that when I used my laptop charger to power the device the fan started to work normally. I see the fan spinning up when I get the thermal trigger.

This issue is happening with my unit too,
the fan’s power is wired to the 5V GPIO on the board.
Measuring the 5V GPIO while the fan is supposed to be spinning reads only 3.5V.
I’ve also fixed the issue by using a different charger
